What is Form IL-1040-X?

Quick answer: Form IL-1040-X corrects an Illinois individual income tax return you have already filed — to fix income, exemptions, credits, or payments, or to report a change the IRS made to your federal return. You complete every line with the corrected figures, reconcile against what you already paid or were refunded, and explain the reason for the change. It follows the same flat 4.95% rate and structure as the original IL-1040. There is no filing extension for an amended return, and a refund claim generally must be filed within three years. Below, the Form IL-1040-X instructions are explained one line at a time, with the current Illinois figures.

Form IL-1040-X is the amended version of the IL-1040. You use it when a return you already filed was wrong or is now out of date — you left out income, missed a subtraction or credit, used the wrong filing status or residency, or the IRS adjusted your federal return in a way that changes your Illinois tax.

The form repeats the IL-1040’s structure in a single “corrected figures” column, then adds steps that compare the corrected tax to what you originally paid and were refunded, and a section where you state why you are amending and attach proof. Completing it correctly matters because the use tax line is frozen at the original amount, a federal-change amendment has its own timing rules and required attachments, and interest runs on any additional tax from the original due date.

Key Figures — Tax Year 2025 (Form IL-1040-X, revision R-12/25)

  • Illinois income tax rate: 4.95% (0.0495), the same as the original return.
  • You cannot change the use tax from the amount reported on your original return (Line 21).
  • If your change comes from a federal adjustment that creates an overpayment (or a federal NOL carryback), do not file until you receive the IRS federal finalization notice.
  • Refund window for a state-only change: the latest of three years after the extended due date, three years after the date you filed the original return, or one year after the date you paid the Illinois tax.
  • Refund window for a federal change that decreases your Illinois tax: within two years plus 120 days of the federal finalization date.
  • A federal change that increases your Illinois tax: file Form IL-1040-X and pay within 120 days of the federal finalization date to avoid a late-payment penalty. Attach a copy of the federal finalization.
  • Mail Form IL-1040-X to: Illinois Department of Revenue, PO Box 19047, Springfield, IL 62794-9047.

Who Must File Illinois Form IL-1040-X?

Anyone who needs to correct a filed IL-1040, including people who: – omitted income or reported it incorrectly; – missed a subtraction (such as retirement income) or a credit (such as the property tax credit); – used the wrong filing status, residency, or exemption count; – need to report an IRS adjustment or a federal amended return that changes Illinois tax; or – are carrying back an Illinois net loss the instructions permit.

Who should not file it. If you have not yet filed an original IL-1040 for the year, file the IL-1040 instead. If the only change is a math error the Department can correct, wait for the Department’s notice. If a federal change created an Illinois overpayment, wait for IRS acceptance before filing.

How to File Illinois Form IL-1040-X

  • Electronically: An IL-1040-X can often be filed through MyTax Illinois or approved software; check the current options for the year you are amending.
  • By mail: Print the amended return, attach the required documentation, and mail it to Illinois Department of Revenue, PO Box 19047, Springfield, IL 62794-9047.
  • Payment: Pay additional tax through MyTax Illinois, or by check payable to “Illinois Department of Revenue” with Form IL-1040-X-V.

Before you start, get a copy of your original IL-1040 and any prior IL-1040-X for the year, your corrected federal return and any federal Form 1040-X or 1045, the federal finalization notice if the IRS changed your return, and any schedules or W-2s/1099s that support the change.

When Is Illinois Form IL-1040-X Due?

  • File as soon as you discover the error. Interest accrues on additional tax from the original due date, so delay is costly.
  • To claim an additional refund (state-only change): file within the latest of three years after the extended due date, three years after the date you filed the original return, or one year after the date you paid the Illinois tax. A refund on a claim filed within the three-year window is limited to payments made in the three years and six months before the claim.
  • For a federal change that decreases your Illinois tax: file within two years plus 120 days of the federal finalization date, and attach the federal finalization.
  • For a federal change that increases your Illinois tax: file and pay within 120 days of the federal finalization date to avoid a late-payment penalty.
  • No extension: There is no separate extension for an amended return.
  • Federal-change overpayment: Do not file until the IRS has formally accepted the federal change.

Where to Mail Illinois Form IL-1040-X

  • Mailing address: Not printed on the form. Mail Form IL-1040-X to Illinois Department of Revenue, PO Box 19047, Springfield, IL 62794-9047 (a single address, unlike the IL-1040).
  • What to attach: a copy of your federal finalization (for a federal-change amendment); a copy of federal Form 1040-X or 1045 if you filed one; every corrected or newly required Illinois schedule; W-2s or 1099s if withholding changed; and your detailed written explanation from Step 11D.

Line 24: Total tax from Line 23. Line 25: Illinois Income Tax withheld — corrected total; attach Schedule IL-WIT. Line 26: Estimated payments from Forms IL-1040-ES and IL-505-I, including prior-year overpayment applied. Line 27: Pass-through withholding — from Schedule K-1-P or K-1-T. Line 28: Pass-through entity tax credit — from Schedule K-1-P or K-1-T. Line 29: Earned Income Tax Credit — from Schedule IL-E/EITC, Step 4, Line 9. Include payments made with the original return and any later payments, per the instructions. Line 32: Total payments and refundable credit — add Lines 25 through 31.

Step 9 — Corrected Total Overpayment or Underpayment

Line 33: Adjusted overpayment — if Line 32 is more than Line 24, subtract Line 24 from Line 32. Line 34: Adjusted underpayment — if Line 24 is more than Line 32, subtract Line 32 from Line 24.

Step 10 — Adjusted Refund or Amount You Owe

Line 35: Original overpayment. Enter the overpayment shown on your original IL-1040, Line 32, or as later adjusted by the Department. Do not include any interest you received.

Line 36: Overpayment. If Line 33 is more than Line 35, subtract Line 35 from Line 33. This is the additional refund you are claiming.

Line 37: Refund amount and method. Enter the part of Line 36 you want refunded, and choose direct deposit (with routing number, account number, and account type) or paper check.

Line 38: Applied to estimated tax. Subtract Line 37 from Line 36 to apply the rest to estimated tax.

Line 39: Amount you owe. If Line 33 is less than Line 35, subtract Line 33 from Line 35. If you have an amount on Line 34, add Lines 34 and 35. If Lines 33 and 34 are both zero, enter Line 35.

Step 11 — Amended Information

A — Reason for the change. Check the box that applies — Federal change accepted on [date], NOL accepted on [date], or State change — and attach a copy of your federal finalization for a federal change.

B — Original filing date. Enter the date you filed your original IL-1040 or your most recent IL-1040-X for the year.

C — Federal Form 1040-X or 1045. Check Yes or No. If Yes, attach a copy.

D — Explanation. Explain in detail every reason for the amendment. Attach a separate sheet if needed.

Step 12 — Signature

Sign and date. Both spouses must sign a joint amended return. A paid preparer completes their section; complete the third-party designee section if you want someone else to be able to discuss the return with the Department.

Common Mistakes to Avoid

  • Changing the use tax. Line 21 must match your original return.
  • Filing a federal-change amendment too early. Wait for the IRS to accept the federal change if it creates an overpayment.
  • Leaving Step 11D blank. The Department needs a clear, detailed reason for the change.
  • Not attaching the federal finalization. A federal-change amendment is incomplete without it.
  • Forgetting Line 35. You must back out the overpayment already refunded on the original return, or you will double-count it.
  • Missing the refund window. A state-change refund claim is denied after the latest of the two three-year limits or one year after payment; a federal-change refund claim after two years plus 120 days from finalization.
  • Amending before the original return is processed. Wait until the original IL-1040 has been accepted.
  • Not attaching corrected schedules. Any schedule whose numbers changed must be included.
